This PPO plan utilizes the Ameritas Classic Network, reduces high out-of-pocket costs, and graduates to our highest annual max after 3 years on the plan.||The Choice 1200/2500/5000 plan offers the same benefits as the Network version of this plan but allows you to pay less out-of-pocket while going out of network. This plan also graduates to our highest annual max available ($5,000) after 3 years on the plan.||One of our most popular plans, this plan offers the high $3,500 annual max with no waiting periods so you can get the best value for your monthly premium. This plan utilizes the Ameritas Classic Network and reduces you out-of-pocket costs.||The Choice 3500 plan offers the same benefits as the Network version of this plan but allows you to pay less out-of-pocket while going out of network. This plan is a great option if your dentist does not participate in networks.||This inexpensive policy offers the most network coverage if you're in need of coverage now, with no waiting periods. This plan gives your the most coverage at the lowest price, and utilizes the Ameritas Classic Network.||The Choice 1200 plan offers the same great benefits as the Network 1200 plan but allows you to pay less out-of-pocket while going out of network. We recommend this plan if you need coverage now for major services, but your dentist does not participate in networks.|

How It Works
- Sign up for Spirit individual dental insurance online. It only takes a few minutes.
- In about 10 business days, you’ll receive your ID card in the mail.
- On the effective date of your policy, you can start using your insurance for cleanings, checkups, and major services. Just let your dentist know that you have coverage, and you’re all set!
- Coverage is available right away. You will be able to start using your plan the first day of your effective date.
- Coverage options differ per plan. Make sure to compare before purchase.
- Guaranteed acceptance.
- All plans have a monthly premium and a one-time $100 deductible.
- Your monthly premium will be based on several factors, including your zip code, the type of coverage you select, and whether you opt for a Choice or Network plan.
